[___] \u2013 Loss of [date]<\/strong><\/p>\n<p>Sir or Madam,<\/p>\n<p>On [date], I suffered a loss covered by my policy no. [___]: [describe: water damage \/ theft \/ fire \/ ___]. I notified you on [date] and sent you my proof of loss and supporting documents on [date].<\/p>\n<p>More than 60 days have passed and you have [not paid the indemnity \/ refused coverage without valid reason \/ offered an insufficient amount of $___].<\/p>\n<p>Under article 2473 of the Civil Code of Qu\u00e9bec, the insurer must pay the indemnity within 60 days of receiving the statement of loss.
