Re: Claim no. [___] – Loss of [date]
Sir or Madam,
On [date], I suffered a loss covered by my policy no. [___]: [describe: water damage / theft / fire / ___]. I notified you on [date] and sent you my proof of loss and supporting documents on [date].
More than 60 days have passed and you have [not paid the indemnity / refused coverage without valid reason / offered an insufficient amount of $___].
Under article 2473 of the Civil Code of Québec, the insurer must pay the indemnity within 60 days of receiving the statement of loss. I therefore put you on notice to pay the sum of $[amount] within 10 days, failing which I will institute proceedings for the indemnity, interest and damages, without further notice [and file a complaint with the Autorité des marchés financiers].
