Re: Psychological harassment at work

Sir or Madam,

Since [date], I have been subjected by [name, position] to repeated vexatious conduct: [describe: humiliating remarks in front of colleagues on ___, unjustified exclusion from meetings, threats, excessive monitoring]. This conduct undermines my dignity and my psychological integrity [and has led to sick leave from ___].

I reported the situation to [name] on [date], without effective measures being taken.

I put you on notice to take, within 10 days, the reasonable measures required by section 81.19 of the Act respecting labour standards to put an end to the harassment, including an investigation by an impartial person. Failing this, I will file a complaint with the CNESST and exercise all my recourses.
